Summer Camp Registration
This form is for registering children to attend Summer Camp at St. Stephen Protomartyr Ukrainian Catholic Church. Please fill out a new form for each child you wish to register.
Summer Camp Youth Volunteer Form
This form is for youth (age 12 - 17) who would like to volunteer for St. Stephen's Summer Camp 2024. This is a parental consent form. It must be completed by a parent or legal guardian. Please fill out a new form for each youth volunteering for camp.
Summer Camp Adult Volunteer Form
This form is for 18+ volunteers only. If you are under 18 please have a parent fill out a Youth Volunteer form for you.
