SUMMER CAMP REGISTRATION This form is for registering children to attend Summer Camp at St. Stephen Protomartyr Ukrainian Catholic Church. Please fill out a new form for each child you wish to register. SUMMER CAMP YOUTH VOLUNTEER FORM This form is for youth (age 12 - 17) who would like to volunteer for St. Stephen's Summer Camp. This is a parental consent form. It must be completed by a parent or legal guardian. Please fill out a new form for each youth volunteering for camp. SUMMER CAMP ADULT VOLUNTEER FORM This form is for 18+ volunteers only. If you are under 18 please have a parent fill out a Youth Volunteer form for you. Registration and Payment can be completed online at https://tinyurl.com/bddjvvp7 Half day: $50 Full day: $100 Family: $250 (3+ children) Please contact Lynsey or Fr. Janko for requests for subsidy
